i have been wondering about combining hair transplant and hairline lowering procedures in males with dht-driven hairloss

this post explores this possibility.

a hair transplant procedure involves extracting follicles from the back of the head in the dht-resistant zone and reattaching them in the front part of the hairline or the crown when dht-susceptible hair has been lost. it may leave linear or spot scarring at the donor sites, no scars on forehead.



a hairline lowering procedure involves removing a portion of the upper forehead scalp and bringing it down towards the brows. it leaves a linear scar along the the hairline that is masked by hair follicles. a hair transplant of a few hundred follicles in front of the hairline post-lowering may help hid the linear scar further. the portion of the scalp itself with hair does not change, so the density of the hair remains the same. for those with no hair loss, this is desirable, because hair transplants cannot achieve the same level of density that natural hair offers.



hair transplants are performed on both females and males.

hairline lowering is typically performed on females only or males with zero risk of further hairloss (eg trannies on hormone therapy). this is because males may experience further hairloss that results in the hairline scar becoming visible.

Good candidates for hairline lowering surgery include women with a naturally high hairline or large forehead who otherwise have thick hair and no personal or familial history of hair loss. Men with a high hairline or large forehead can also be good candidates for hairline lowering surgery if they have a thick hairline and no personal or family history of hair loss. Good scalp laxity and hair density is important, and therefore, a detailed consultation and examination with Dr. Champagne is necessary.


the thought that occurred to me is the following: IF a male gets hair transplants to replace their entire hairline with dht-resistant hairs, THEN AFTER is it safe perform a hairline lowering procedure as well?

the reasoning is that you know the transplanted hairs aren't going away, so maybe forehead lowering is ok.

from my research there doesn't seem to be much discussion of this except here

these doctors seem to think it's ok

speculative reasons NOT to get hairline lowering even after transplantation:
1. transplanted hair density is insufficient to cover the hair line lowering scar
2. further hairloss may still occur, so eg what if you want to shave your head someday and the frontal strip is visible then?
3. eyebrows get raised and look more elevated (bad for males)

apologies, should have been more clear about that. you can save donor grafts for eg crown restoration by doing hairline lowering instead of transplanting the entire area needed to lower the hairline up front. it would take 1000s of donor grafts to mimic 1inch hairline lowering.

but having to get repeated transplants for further balding that happens down the road sucks in itself, just take fin tbh

for sure. fin slows the reaper but does not stop it. even with fin you may need transplants in 40s, 50s, etc

until then we should really have something else tbh
 
Eyebrows do not typically get raised during this procedure. The tension is coming from the back of the head, not the front.

Nah fam u get scalp advancement first and then get hair transplanted into the scar to cover it and then hop on ru + dutasteride + minox + microneedling and boom we’re taking our NW0 to the fucking grave
