Your base looks good but you look like you don't do f' all to take care of yourself. You look bloated and your skin looks like pepperoni. Underneath your face looks robust and you have some decent facial features. Lose some weight, hop on a skin care routine (your acne isn't the worst but it's noticeable so maybe hop on some tret), put on some sun screen and get yourself some sort of a tan, that or eat some carrots because you need a little bit of colour in your skin because right now your skin looks really dead and is of a ghoulish tone.
Don't bother with surgery right now, you have a decent base that can be worked with through simple softmaxxing. If I were to give any sort of surgery suggestion it might be a lip lift because your upper lip looks a bit thin in comparison and your philtrum looks only a tad elongated. But really, just softmax first and then worry about surgery. (which I'm pretty sure you don't really need)
