This is what I would look like AFTER double jaw surgery aka bimax with 7-8mm of upper and lower jaw advancement + 6degrees of CCW

My surgeon did this morph

As you can see, even in this after, I STILL LOOK LIKE SHIT :feelscry: :feelsrope:

I SWEAR TO GOD, MY JAW RECESSION, NARROWNESS, AND SHAPE ARE ALL THE REASON WHY IM ON HERE

MY LOWER THIRD IS LEGIT 0/10, IT FUCKS MY FACE SO MUCH

I might as well wrap a rag with superglue to cover my lowerthird, and get rhino/cheekbone implants for nose to fix my mid face, and then ascend by betabuxxing in Iran for a wife

Is bimax even worth it if this is my after result?

After double jaw you could do implants. Don't do them with a recessed maxilla/mandible though.

I’ll make a gif with a superimposition of the after onto the before, to highlight the changes, tomorrow.

But, you have to keep in mind that profile is the least important, when considering 3/4 and frontal.Those should hugely improve.

Yes your front looks good your side is utter horseshit but the bimax will give you an average side that you can always improve more later on. This now will at least give you liveable life.

this surgeon says he can give me 7.5mm's on both jaws + 6 degrees of CCW without touching my chin

but I want to find a surgeon that can do 10mm's on both jaws instead of 7.5mm's

Yeah ones in Miami and the other in Australia. There’s another in Dallas but he seems less likely to do it on non deformed patients.

If you can find a surgeon who will give you more advancement go for it. But didnt you already say before that most surgeons wont do that big of a change? Look for another if your confident you can find a surgeon who can do more tbh but that is already a huge improvement

Just straighten yourself up, like you want to stand as tall as possible.
But the hard part is remembering to hold good posture, it's all subconscious.

Yeah.
Through osteotomy, you’d likely need bone grafts, and a surgeon even willing to try it on you (the two I mentioned being able to do Lefort IIIs on cosmetic cases would likely do so)but it would very tricky.

Through Implants it’s totally doable (Eppley has done it), but I imagine you’d still need a coronal incision, unless it’s made of silicone and separated into small pieces.

Here’s an example of a custom one Eppley made to a patients Specifications (note if you wanted it in something like PEEK, a coronal incision is almost definitely needed)

I guess personal preference against implants there. And certain implants like silicone can cause bone erosion.

For instance, in my case, one orbit is slightly less developed than the other, causing a depth asymmetry. Even if I got Orbital decompression in that eye, the rim sorrounding isn’t developed as much as the other. So I can either get an OBO to fix it, or implants. Personally, I’m still undecided, since the OBO doctor I’ve spoken to is very confident in being able to address it. And my personal preference is, to try to fix things with osteotomies, and only got for implants afterwards, but this is the case for me because I can afford it. If not. I may cut corners with implants there instead.

Are you not scared about the risk of nerve and tissue damage in the eye area? Although it may be annoying near the jaws, I’d probably kill myself if I were blind.

Yeah but the doctor who would do it is among the most experienced craniofacial surgeons ever. Likely top 5-10. He said that he could have a neurosurgeon present if it makes me feel more comfortable (since your brain is exposed, and another doctor who I spoke to requires a neuro to be present), but that he doesn’t need one because they come to him for advice all the time. He’s also done one of the highest numbers of Lefort IIIs.

Hm I believe I know who you’re talking about then lol. Have you seen the case of the person on jawsurgeryforums who got a MFL3 . He stated that there were very subtle differences that didn’t really result in a remarkable different and never reported back on complications.

How do you resolve the disbalance of top and bottom jaw? Do you need to undergo another surgery that moves bottom jaw forward? Doesnt lefort 3 include lefort 1 area?

@Short Ugly and Brown after looks fine, i think profile just needs to look acceptable and not resseced, after that it doesnt matter that much, how many mm's do you think the rotation will take away from your philtrum?

@Golden Glass does it make sense to do imdo and bsso after for more advancement? i would need 1.5cm+ in my lower jaw, its very recessed, would that be possible?

I more meant to ask, if you do lefort 3 then wouldnt you also automatically need bsso? As you are advancing the top jaw forward? That's assuming you have a stable occlusion at the start of lefort 3 (like in the case of op).
