A question for Dr @RealSurgerymax if you will be so kind sir

What is your opinion of Ramus Widening during a Bimax or BSSO? Is there a scientific consensus on whether it is safe? Does it depend on the initial position of the condyles?

I've heard/seen some surgeons do it (Pagnoni, Augusto Pary) and others say it is a bad idea (Ramieri). It seems like a perfect antidote for narrow bigonial width

  • It can be done minimally (almost unnoticeable) through condyle torquing, which can occur in varying degrees and rotations depending on the angle of the BSSO cut. The angle of the BSSO cut deoends on the width of the bone & location of the neurovascular foramen.

  • It can be done through adapting the mandible to straight or bent-out plates instead of adapting plates to a curved-in mandible

  • It can be done by sandwiching a bone graft between the BSSO segments

• Non of the above methods will result in male-model “flaring” jaw angles. CUSTOM Implants are still best for aesthetic jaw angles.

He's based in Cambodia and bimax's are only a hobby. His main income is from owning a brothel.

Ahhh the ramus widening... yes the first thread is mine! To be honest when I got the bimax with Pagnoni I never thought to widen the ramus but he said with the additional forward growth my face would look too narrow and that would balance it out.

Curiously enough, Ramieri is doing jaw widening at the moment (a user on .org got it @politically correct )

Do you have more information on what Ramieri said exactly? Why is it a bad idea? Did he change his mind? Or did he discover a new method?

Would ramus widening improve cheekbones/hollowness?

Ramus widening is more limited in creating hollow cheekbones since the entire surface of the ramus is moving out unlike an implant which can make a more concave space.

But it would lead to an improvement nonetheless, if I understand correctly?
