You definitely need bimax with ccw rotation and maybe some chin work done on top of it. As for the eye area, your eyes are protruding due to lack of surrounding bone structure, you might be able to fix it with infraorbital rim implants and orbital decompression.

Can't conclusively tell how fucked you are since it's difficult to measure all relevant ratios from the pics you sent.

Lefort 2 is idiotic because his nose is roughly where it should be already, as it is apparent by his nasolabial angle. Mlf3 could work but I doubt anyone here is brave enough for that, and I'm not even sure he can get enough change in his eye area from that, i.e. his undereye support could still be lacking and he'd still have too much uee.

Yeah, a lf1 with ccw rotation should fix his nasolabial angle and make his philtrum look better. I think he looks pretty bad if that's his actual mouth width, in which case he might need mse or mouth widening surgery on top of what I suggested.

And yeah, I know, mouth widening can leave visible scars, but better be a decent looking joker than be stuck with cuck lips.
