As your DHT levels lower, your testosterone will try to take over certain androgen related functions that DHT is responsible for as a way of "backfilling" and reaching a homeostasis like state with lowered DHT.
But this may not be the end all be all, as people like myself who are on exogenous testosterone have abnormally high Free T levels however continue to experience side effects.
Remember, you are not only inhibiting testosterone converting to DHT, but many other important functions.
Such as these.

- Progesterone → 5α-Dihydroprogesterone
- 3α-Dihydroprogesterone → Allopregnanolone
- 3β-Dihydroprogesterone → Isopregnanolone

I hypothesize many people's side effects occur because progesterone is unable to properly convert into allopregnanlone, and allopregnanlone is the body's natural antidepressant, it regulates feelings of well-being, anxiety, depression even sexual drive and erectile function.
I think this is an incredibly important thing to consider when using 5ar inhibiting drugs such as finasteride and dutasteride, however all too often it gets swept under the rug and most assume the lowered DHT is the issue and catalyst for sides.
It might be different for certain individuals however, as we all have different cognitive architectures, some people have an abundance of GABA receptors and don't experience as drastic side effects as those with less GABA receptors because their brain doesn't solely rely on allopregnanlone as the body's natural antidepressant. But rather have many GABA receptors to help out in related functions and "take over", sort of similar to how testosterone can "take over" androgen related functions for DHT, when a lack of it is in the body, but not exactly in the same way, but you get what I mean.
Blood work will always help determine the cause of sides, but dealing with finasteride and other related drugs can be incredibly tricky, and It's not worth it, rather just try to cope with some topical anti-androgen, but I don't even know about those at that point, many are either ineffective or have other side effects as well.
I think a hair system might be the best option, tbh.
