Genetic and environmental, you could get them with maxila advancement(aka leforts) or with cheekbone implants which I highly suggest if you have a flat maxila and a long midface

Best option for that is modified lefort 3 imo, because it’s not as brutal and brings significant maxila advancement, lefort 3 and lefort 2 are meme surgeries and almost no surgeon would perform them for aesthetic reasons

However i think i can get lefort 2 or 3 because i have breathing problems which makes it hard for me to breathe with my nose.

theyre definitely genetic. My mom had really good cheekbones so I somehow got them, but neither of my parents have a good jaw, which is why I don’t have one.
