Would be best to have calipers but if you don't you can use measuring tape and mark your width with a pencil on a wall so you measure in 2D.
In worst case scenario you can use ImageMeter but it might not be accurate.
Here's how I've done it but it's not really accurate because I also used calipers and my waist was 28cm across and my bidelt was 56cm giving exactly 2.0 ratio. I'm also holding a phone in my hand which also impacts the measurement.

It doesn't do anything with real length but has a lot to do width perceived length. Person with 2.0 ratio will look wider than a person with 1.6 ratio given they're not standing exactly next to each other (which doesn't happen often on a runway)

depends. obviously working out your shoulders, but some extreme alternatives would be certain steroids or like you said surgery.

Yes, depending on your height of course. Someone that’s 180cm with 43cm may appear relatively broader compared to somebody that’s 190cm with 44cm.
