Buccal fat removal is one of the most overhyped procedures right now. It permanently removes deep cheek fat pads to give a sharper midface. But here’s the issue: as you age, your face naturally loses volume. If you already have low body fat or hollow cheeks, taking out buccal fat can make you look gaunt or older very fast, especially after 30. The ideal candidate is someone with full, heavy cheeks even at low body fat, and strong bone structure underneath to support the contour. Otherwise, you're basically signing up for "skinny face collapse" in your 30s and 40s. If you haven't hit 10–12% body fat and maintained it for a while, you should NOT consider buccal removal yet. Many people think they need it, when in reality they just need to lean down.
