You can die or fuck your life up from this surgery.
It’s when your bite is normal but still short lower jaw.

Zarrinbal has some nice results with them- seems like halfway between BSSO cut and genio but without the need for braces
 
chin wing doesn't change occlusion at all. it's basically just a genioplasty that extends along the whole mandible. results seem hit or miss.

Chingwing is like a more advanced but more risky genioplasty.

These types of surgery (bone manipulations) are risky because it is difficult to get them symmetrical to the nearest millimetre. It’s much easier to achieve a better result with an implant.

You also need to consider that a chin wing/implant is only a cosmetic fix and won’t do anything to fix your functional problems. If you’re going to take the risk and pay the money for a chin wing, there’s almost not reason not to get a bimax instead because it will look better as well as solving any breathing/bite/TMJ problems.

The main drawback of a bimax is that it requires the patient to have perfect teeth alignment before the operation can be performed which will almost always mean the patient needs braces. A bimax is also slightly more likely to cause a loss of sensation.

So could reduce soft tissue bunching

I’m happy to just spend my whole life never doing an open mouth smile at this poiny

I don’t understand how normies with jaw issues are happy to smile open mouth in photos

So if someone has an underbite/overbite they would need perfect teeth before? makes no sense, unless you’re not referring to bite

I should clarify that when I said “perfect teeth alignment” I was talking about straightness, not occlusion (bite).

But yes, a bimax is almost always involves a collaboration with both an orthodontist and jaw surgeon. No Maxfacs surgeon in the world will do the operation if the patient doesn’t have straight teeth before the surgery.

Do you know anything about reducing swelling as fast as possible? Is it even a thing?



She seemed to have lost swelling fairly quickly while others don’t. What does it depend on?

My swelling was similar to hers. Ice and tape aren’t going to do shit for a face which has just been smashed to pieces.

I was given dexamethasone for mine which probably helped lots but most surgeon’s won’t because of the risk of side effects.

I’ve had enough surgery’s to know the swelling is always worst on day 3 and is almost all gone by day 12.

Mine was slightly different in that they harvested bone from my hip crest to graft my maxilla downwards.

As a result I got a lot of “referred peri-umbilical pain“ and couldn’t walk properly for about a week.

It’s the same pain people get from appendicitis because they share the same nerve root.

Honestly the jaw was uncomfortable but didn’t hurt at all. The referred pain was agony when I tried to move my right leg.

Btw how straight do the teeth need to be? Mine are almost perfectly straight just one of my teeth is too far “up” and a slight gap in my lower jaw

I can’t say because it’s not my speciality but the back are more important than the front.

Before the surgery they’ll record a 3DCT scan of your current bite. They’ll use software reposition your jaws to an ideal bite:

They then use the software to 3D print a “splint” which will hold your teeth in the ideal bite position while they attach your jaws back to your skull using titanium plates:

You might need to wear the splint for a week after the surgery with your jaws tied shut with elastics, you might not. I don’t think there’s clinical evidence either way. I did for my first bimax, I didn’t with my second.

Difficult to say, lots of reasons.
The first one was kind of botched even though it was done by a very prestigious surgeon at a very prestigious hospital. It fixed my bite but I was still very recessed and laughably asymmetrical.

I spoke to a few jaw surgeons and I’ve been told that 20% of bimaxes relapse, especially in teeth grinders like me.

I also got my teeth changed to crowns which gave me a perfect bite and set of teeth but apparently affected the direction of forces generated in my jaws which likely contributed.

7 years later I got it done again by a Hungarian Surgeon in turkey who I found on Instagram doing 5-7 cases per week and uploads them all to his daily story.
He’s got a couple of bad reviews but so far as I can tell, he’s the real deal and does practically nothing with his time except jaw surgeries, especially with complicated cases.
