Considering Ramieri for a Bimax - What do I ask him and how specific can I get with what I want?

I've heard that certain surgeons don't appreciate direct requests depending on what they are and I also don't want to mess up my consultation with him.

Can I ask him for specific things in my bimax such as a wider jaw, ramus lengthening, gonion widening (I don't know the exact terms for these words so excuse my lingo.)

I don't want very specific results other than a wider jaw, wider and more forward grown maxilla which would also fix my nose, and outwards gonions to go with my wider jaw.

Let me know so I can get the most out of my consultation.

Ask him for a bimax with ccw rotation

Isn't CCW rotation for editing the midface though? I don't know if my midface needs anything, or is that not what it does?

Its for downward growth

It’s for gonial angle/downgrowth

sounds like you want jaw implants

Ramieri is pretty chill tell him you want wider jaw , more forward maxilla and lower jaw

He will explain everything you want he doesn't rush and take care of the patient interest

100% you can trust him

Thank you! I will take this into account.

Yes, a CCW (counterclockwise) bimaxillary osteotomy can help lower a high gonial angle, but the extent depends on the individual’s anatomy and the surgical execution.

How CCW Bimax Affects a High Gonial Angle

1. Mandibular Plane Angle Reduction – A high gonial angle is often associated with a steep mandibular plane. CCW rotation brings the chin forward and downward, effectively lowering the angle in some cases.

2. Posterior Ramus Positioning – If the surgery includes ramus lengthening (via vertical ramus osteotomy or custom implants if needed), it can make the gonial angle more defined and less steep.

3. Chin Projection & Jawline Definition – As the mandible rotates forward, the gonial angle may appear less acute and more balanced, improving lower facial aesthetics.

4. Soft Tissue Effects – The visual outcome also depends on muscle and fat distribution post-surgery.

Limitations

• If the CCW rotation is too aggressive, the gonial angle may remain high due to excessive anterior rotation.

• If no ramus height increase occurs, the mandible might not have the structural change needed to reduce a high gonial angle.

• Some patients may need additional gonial implants or fat grafting for optimal definition.

Would you like to discuss your specific case (e.g., your current mandibular angle and desired changes) to determine if CCW bimax alone would be enough?

lmao. talking about CCW rotation. posts some shit about jaw implants and vertical ramus osteotomy. you fucking moron. you absolute idiot.

imagine thinking ChatGPT is an accurate source of information too. jesus fucking christ

Can u ask him if it’s possible to elongate a short midface for someone with no recession annd good tooth show and if it affects soft tissue in the outside?

Just completely disregard the first part which is why I sent it I didn’t care enough to delete the rest, fucking dumb piece of shit. I said ccw bimax in my original comment, it obviously changes gonial angle

Yes he can maxilla downgrafts for SFS cels. Ramieri is also very good for that

Nah I meant short midface that’s genetic like u have good teeth show and no recession. Not the sfs where the maxilla is not properly developed
