Is it necessary to undergo a SARPE AND Lefort 1 for recessed chin/jaw + underbite + long midface? I am also wondering the same about the Bimax.

I have issues breathing so my insurance would cover most, if not all of these procedures. I am just wondering the most optimal procedures to fully ascend from normie

you dont need lefort 1 so sarpe and bimax together are good, unless you are very subhuman

Pm pics, your case sounds similar to mine, you have an underbite with a recessed mandible too?

Sounds like you need bimax ccw idk why you’re taking about lefort 1

SARPE (Surgically Assisted Rapid Palatal Expansion) is for fixing Transverse maxillary deficiency (narrow palate) by widening your maxilla and dental arch.
In a LF1 osteotomy they cut this

part of the maxilla and they usually advance it forward (you can also do stuff like downgrafting and impaction to rotate your jaws clock-wise or counter-clockwise) and fix it in it's new position with plates and screws, this is for fixing Sagittal maxillary deficiency.

Bimax (Double jaw surgery) is for fixing both maxillary and mandibular Sagittal deficiency by advancing both upper (Maxilla) and lower (Mandible) jaws forward, they do this by doing and a LF1 Osteotomy and BSSO (Bilateral Sagittal Split Osteotomy) respectively.

There are also different techniques of Genioplasty (chin surgery) which have a much, much easier recovery then BSSO. Combining upper, lower and chin surgery is sometimes called Trimax.

Surgeons also commonly simultaneously fix Transverse maxillary deficiency when doing LF1 or Bimax by doing a Segmental Lefort also known as Two or Three-piece Lefort during the same surgery. I recommend SARPE more, however, although I don't know much about Segmental Lefort.

i heard you need also rotate mandible because you will end up with new malloclusion.
