Does a CCW rotation make a huge difference in terms of aesthetics?

I keep hearing about a CCW rotation when it comes to jaw surgery. Does it really make that big of a difference when you get one in terms of aesthetics?

yes, the rotation of occlusal plane, which may be performed clockwise, counterclockwise or not performed at all, makes a huge difference.
Your maxfac doctor will figure out the best kind of rotation you need

How can the maxilla actually be ccw rotated tho? Is a bone segment removed?
In which case isn't this just for gummy smile cels?

yes, a bone segment is removed to perform the CCW rotation [1]

while your upper maxilla is pushed forward in Lefort 1 osteotomy, the CCW rotation is performed to prevent that your face becomes vertically longer.
if your doctor does not perform the CCW, your face usually becomes vertically longer
I think most guys in the forum underwent bimax with CCW.

for gummy bite cels, the CCW rotation is obviously performed and the specifical orthognatic surgery is called "Lefort 1 impaction" [2]

Ye I thought impaction and bone removed was just for people with gummy smile/maxillary excess, otherwise the soft tissue will be too long for the bone


I always wonder why they can't just cut like this: (sloped cut so lower maxilla slides forward at an upwards angle, rather than straight cut for just forward movement)

CCW is huge for birdcels. With normal mandible advancement you are limited by your bite, if you have 5mm overjet you can't really go for more movement than that, with CCW it is possible. It works in such a way that surgeon changes your occlusal angle, thanks to that mandible can be rotated up and forward

There are two ways to do CCW, you can cut bone in front of maxilla through impaction or add more bone in the back through downgraft. Downgraft is more popular nowadays and you can't do impaction without gum show anyways

You are good candidate but you need to post ceph for more info. CCW is tricky and very dependent on your occlusion, for example if your occlusal angle is flat like mine you can't really get much CCW or you will end up with reverse smile. Different surgeons use different orientations and results may vary so its important to pick a good one

yes, ccw rotation promotes more chin advancement in general, but u need to take into account ur oclusal plane angle so u dont get a inverted or neutral tilt smile (very uncanny). also some people might need cw, wich in turn gives less chin advancement but more chin height.
