I would say a dbj surgery with cw rotation but that might fuck ur midface up

Going to have to disagree, this guy had barely any jaw mass yet jaw shaving + facelift ascended him hard, I also plan to get this.

Op's bigonial width is within the ideal, and his lower jaw is actually quite short in comparison to his long midface. Mandibular shaving in op's specific case will only make it worse for him I'm afraid

Honestly OP there's not much you can do, your FWHR is like 1.76 in a non-distorted picture which is well outside of the ideal.

His jaw width looks cucked now, plus he's going to age prematurely with less bone to support his lower face, he would've been way better off with facelift and something to add height to his chin

The after pic is also frauded. Heavy nasolabial fold disappeared, under eye bags disappeared, better hair, even the lips look tweaked and improved.

I do see the jaw shape change but the ascension is a lot of things happening at once imo.

Edit - just noticed you mentioned he also had a facelift, so that could explain the nasolabial. Still looks a bit sus to me though.
