Getting Bimax in 3 weeks

I'm planning it next week with Dr. Dr. Kater, but the plan is to only get bimax + genioplasty. He is a very good surgeon but i'm scared, that he's too conservative with movements for looks. I'm paying 100% privat, i'm 17 and i want to do "risky, larger movements" and maybe get a ccw rotation and maybe even zygoimplants. I don't know how to tell him tho, because as i said i'm still 17 and i'm scared that he won't give me the bimax if i want "too much". Any ideas or tips for me?

just cosmetics, the german healthinsurance checked the case, but they couldn't find enough medical flaws to cover the surgery

Everyones jaw grows some untill 20-22, not so much that it matters for looks but it matters for surgery
