How do I bring up facial bone growth up to my dentist 15M

I’m going to the dentist tmrw and as a consistently rated ltn I want to ascend. What can I ask him about or bring up to try and get some sort of appliance to help me. Do I just ask him point blank or is there a way for this or to fake it. Give advice on what needs help
