How do they widen mandible if they do three-piece lefort?

I have overbite, but mostly because of the front teeth, back teeth more or less align.

My both jaws are too small but similar size, maybe top jaw is a bit bigger.

Getting both jaws wider would probably help with aesthetics but also more place for tongue and airway.

Surgeon plans to do three-piece lefort (to rotate the front teeth more maybe), so it would make the upper jaw wider also?

But how would the surgeon widen the bottom jaw (and bite, for bite alignment?)?

I must be misunderstanding something, right?

they normally don't, or widen the teeth with orthodontics, but it is possible to do a segmental mandible surgery

I don't know anything more about it than that it is done, but, yeah, it seems like it would carry a greater risk of nerve damage.

mandible distraction osteogenesis is the only way to physically widen the mandible, however it MIGHT cause asymmetric bone growth, if you dont want that, jaw implants is your best bet here.
