How do you maintain friends after surgery?

I plan on getting surgery in the summer after I finish uni. However, I don't want other people to know, so I'm unlikely to be able to see current friends.
I also have one friend from school who I want to maintain.
Do I get surgery, not see them for a while, then continue to be friends, or just stop and move on to new people? Uni friends will be wondering why I stopped seeing them, and I will be sad to lose my school friend.

I could just get surgery and continue as normal and stuff, but then I'll be judged

I am actually what most people would consider very handsome. A random woman at work told me "your eyes are gorgeous by the way" the other day

And I am against surgeries now. I was before too. I had a 2-3 year period where I was brainwashed by this forum

yeah sure that's why you planned all the surgeries in the book.
But you found out that with your zero work ethic, you'd save for it until you're 57. so you preferred to solve your inceldom by escaping to the realm of delusion
