How is hairline lowering performed?

Question for a 5head hairline (no spot balding), is the skin pulled down a little? Or a hair transplant is performed? Which option is better?

I'm not 100% on this but basically they take your foreskin and they encapsulate your whole face in it like being shoked with a plastic bag and you die from suffocation
