Well honestly, I think you'd do great on Accutane, it has a lot of side effects but if you use a correct dose until the acne is gone you'll do fine. You don't need to continue using Accutane once your acne is gone thankfully.

The only other solution I know of is Spironolactone, but that's usually only prescribed to women because of the hormonal side effects, it's partially anti-androgenic, so it will reduce sebum production in the skin among other things. But as a man, I don't think you'd be interested in something that's occasionally used in transgender treatment...
Accutane is the best thing for cystic acne and was designed for this.

it has a lot of side effects, + i am not ready for purging phase
Side effects are tolerable for most unless your on high doses. Every retinoid has a purging phase just deal with it. Go to a dermatologist and talk with him.

Being 100% honest with you, I think you should just to Accutane right away, and then consider continuing Tret or Taz, in almost all cystic acne cases topicals are not enough.

I'd use both benzoyl peroxide (to kill bacteria and reduce inflammation) and Accutane.

okay bro, i'll see a derm for prescription, but i am worried about the hormonal side effects of accutane, someone was larping on this forum saying it causes hairfall, may decrease testerone and stuff

Im completely sure it won't make your hair fall off, but yeah, Accutane does decrease testosterone slightly, it's a non-issue really, would you prefer to have horrible cystic acne and scarring or have slightly less testosterone for a few months? Testosterone aint allat, get rid of your acne and then you can worry about it.

That aside, if you're really worried about the side effects and your derm prescribes Accutane then you can tell them that and they may prescribe you a low-dose as opposed to the regular amounts, that'll minimize the side effects, at the cost of slower results, but if your case isn't that severe, it could work

Tretinoin 0.05% is a solid choice, but those deep breakouts might mean you’re in the purge phase or need something extra. Try using a gentle benzoyl peroxide wash in the morning and keep tretinoin to 2–3 nights a week for now. Make sure you're not over-exfoliating with too many actives. Get Tret from dermacarehub.com—they’ve been super reliable for consistent, prescription-strength help.
