If ortho said I need orthognathic surgery for bilateral crossbite, what does that mean?

Like it's not that bad, maybe 4-5mm on each side asymmetrically, but she said my upper arch was smaller then my lower arch, so wouldn't that mean MARPE and not Lefort 1 osteotomy with BSSO? If I get that, my upper palate will still be too small.

Your issue is called skeletal mismatch (not just misalignment), it requires le fort to reposition the maxilla and bsso to do the same with the mandible.
Marpe is restricted and can only widen the palate to some degree.

The mismatch is noticeable, but would a procedure like this widen by upper palate, or just readjust my jaw alignment?

It'd just reposition it. Marpe/sarpe is still required if you want more width
