But if I remember correctly a high cut lefort 1 does not solve the lack of under eye support problem.

They are not as big as boob implants (jfl) and undereye area is supposed to be soft :lul:

A modified lf 3 doenst cover the whole area of a lefort 3
 
In general I advise the modified LeFort III over the “High (Infraorbital) LeFort I” for 3 reasons:

1.) High LeFort I (up to the Infraorbital's) may leave irregularities like step-off deformity especially in the large moves desired here for noticeable aesthetic change.

2.) High LeFort I will augment the midface more medially but not laterally (as the LF3 would) is not really an aesthetic contour in most people unless they have hypoplasia very specific to this area. So this cut could actually make the malar eminence and zygomatic arch look even weaker.

3.) And let me point out there is no difference in safety or recovery just because one is called a LeFort I and the other is called a LeFort III. A high LeFort I is arguably closer to a LeFort III than a LeFort I and a modified LeFort III or Kufners Osteotomy is closer to a LeFort I than a standard LeFort III. These are just names the cuts are similar no matter what with the same risks and recovery. Don’t let the “3” scare you into thinking it’s going to be much worse and don’t let the “1” lead you to believe this is going to be a walk in the park either.

There are several modifications of the LeFort III and many surgeons have their own. So “modified LeFort III” isn’t really a specific term.

Some so-called modified LeFort III’s are really just malar osteotomies and do not move the teeth or the mouth so not a LeFort at all.

Some move the teeth and mouth but don’t involve the nose (which can lead to a central midfacial hypoplasia appearance if you’re not careful in selection)

Most modified LeFort III’s don’t go as deep into the orbit.

A “standard” LeFort for syndromic correction makes the cut within the orbit, to the orbital fissure. It also goes over the nose.

Look up Kufners Osteotomy, one of the more “standard” Modified LeFort III’s (and interestingly some modifications of this modification are really just “high cut LeFort I’s”.)

Great info, thanks. Is a high-yet-noninfraorbital LF1 (is that what's called quadrangular LF1?) more risky than the standard LF1 cut?
 
Lefort 1 because my upper maxilla is developed decently. At least way better than my lower maxilla and mandible

Is having both an extended LF1

for advancement and a classic LF1 for CCW possible ? Can't get CCW on a high LF1 but still need the projection of the zygomatics..
