1. Sexual Side Effects (Most Common)

These are the most frequently reported side effects, occurring in a small percentage of users, but they may persist even after stopping the drug (Post-Finasteride Syndrome).

Decreased libido (lowered sex drive)

Erectile dysfunction (difficulty achieving or maintaining an erection)

Reduced semen volume (decreased ejaculate amount)

Delayed ejaculation

Complete loss of libido (no sexual interest at all)

Weaker orgasms or inability to reach orgasm

Reduced sensitivity in the penis

2. Psychological & Neurological Side Effects

Depression (ranging from mild to severe, including suicidal thoughts in rare cases)

Anxiety & panic attacks

Brain fog (difficulty concentrating, forgetfulness, slower mental processing)

Mood swings & irritability

Insomnia or sleep disturbances

Fatigue & lack of motivation

3. Physical & Hormonal Side Effects

Gynecomastia (male breast enlargement, may be permanent in some cases)

Breast tenderness or pain

Testicular pain or aching

Reduced testicle size (atrophy in rare cases)

Lowered testosterone levels (although finasteride primarily affects DHT, some men experience lower testosterone overall)

Higher estrogen levels (can contribute to feminizing effects like breast growth and fat gain in hips/thighs)

4. Physical Side Effects

Weight gain (due to hormonal shifts, especially increased fat in the chest and abdomen)

Loss of muscle mass or difficulty gaining muscle (due to reduced DHT levels)

Chronic fatigue or low energy levels

Cold extremities (hands and feet feeling colder than usual)

5. Dermatological Side Effects

Rashes or skin irritation

Increased oiliness or dryness of the skin

Acne (some users report worsening acne, while others experience improvement due to lower DHT levels)

Increased body hair shedding (paradoxically, some men report losing body hair instead of scalp hair)

6. Cardiovascular & Metabolic Effects

Dizziness or lightheadednes

Low blood pressure (hypotension, leading to dizziness or fainting in rare cases)

Heart palpitations or irregular heartbeat (rare but reported in some cases)

Changes in cholesterol levels (increase in LDL “bad” cholesterol in some users)

7. Post-Finasteride Syndrome (PFS) - Long-Term & Persistent Effects

Some men experience persistent or even permanent side effects after stopping finasteride. This condition is known as Post-Finasteride Syndrome (PFS), though its existence is still debated in the medical community. Reported persistent effects include:

Permanent erectile dysfunction

Loss of libido that never recovers

Chronic fatigue & lack of motivation

Ongoing depression, anxiety, or suicidal thoughts

Severe cognitive impairment (“brain fog”)

Persistent gynecomastia (breast growth that doesn’t go away even after stopping the drug)

8. Rare & Miscellaneous Side Effects

Testicular shrinkage (atrophy in extreme cases)

Infertility (lowered sperm count, reduced sperm motility, or abnormal sperm morphology in some men)

Tinnitus (ringing in the ears, rare but reported)

Dry eyes or changes in tear production

Gastrointestinal issues (nausea, diarrhea, or bloating in rare cases)

Nerve pain or tingling sensations (very rare but some users report neuropathy-like symptoms)

Increased susceptibility to sunburn (some users report more sensitive skin)
