I know it’s basically the only way to properly widen the mouth but I haven’t really seen any good results from it, has anybody? If so what surgeon would be best

Eppley still does it in some cases, and I think there is another surgeon in Texas but they can get only a few mm on each side max, which doesn’t have any massive effect.
Ultimately the entire area is about perception.
If your nose is wider than your ICD, getting an alar base reduction will help harmonise the face.
Female patients can get jaw reduction too so that the gonial width is lower and the lips have a more prominent role in the face. I think it can be indicated for some males too if done correctly. Going overboard with fillers or implants for width when your lips are cooked is unwise and your lips look more like an asshole.
Further, bimax can create the appearance of wider fuller lips in specific circumstances. I don’t understand how this happens but I’m meeting my surgeon to discuss it more.
I've been thinking of getting one too with the intention of widening my mouth.
