No. It Wont lengthen your midface. It will only make your lowerthird longer and gives you a visible smile.

Yes. In essence, you want bimaxillary advancement with clockwise rotation.

Short face growth patterns are amongst the hardest craniofacial issues to resolve, and even with major surgery you're probably never going to lost the 'short face' vibe. The whole growth pattern is much deeper than just the height of the mandible.

I have a short face growth battern (strong brachycephalic with skeletal deep bite) however it doesn't look bad at all when I let my lower jaw sit naturally relaxed. It's only when biting that my face looks awkward. Short face folk tend to hold a lot of tension in their masseters due to their growth patterns so hold a more 'closed' bite naturally compared to those with class 1 or class 3 skeletal patterns.

A functional ortho did some work with me to relax the tension I was holding in my lower jaw to find it's true position. Once we'd found it, he determined that it was probably best I got surgery to bring forward the jaws, even though according to my cephs showed that in relative terms i'm within normal range for retrusion.

Make sure your ortho knows their shit about skeletal patterns. Go see several maxillofacialists if you can. A lot of short faced folk get told that they're class 1 dentally on the molars when in actual fact they're physiologically closer to class 3 dentally. This is the issue I have - my upper jaw is just barely retruded relative to my lower one, so I have to 'pull in' my lower jaw to make a class 1 bite. No less than three orthos I saw tried to convince me that this 'pulled back' position was the natural one. It's not. However, they only say this as they know that they're limited in their ability to make significant change sagittally to your upper teeth and 'unlock' the true position of your mandible.

Many orthos will tell you it is purely an aesthetic issue. Yes, short face growth patterns are purely aesthetic, but it's highly unlikely that you have one without skeletal or dental irregularities which have more than purely aesthetic implications.

As I'm in the UK though it's been hard to find a surgeon that is willing to work with me due to the comparatively mild retrusion I have. It 100% would not be covered on the NHS, who only cover jaw surgery if you're legit deformed. And the private market for jaw surgery here is quite small, simply because the British don't feel like paying for any healthcare beyond the shit they get free. I imagine in the US you might find more surgeons willing to operate on you and give you advice.

It's a shame that short face growth patterns are rarely discussed on here. They were rarely discussed back on Lookism either. Everyone seems to be a long faced mouth breather.

I have a short face as well. You will need a bimax and genioplasty or a lefort 1 and imdo.

Your top teeth do not show as your maxilla is too short so a lefort 1 will bring it down.

Also Your mandible and chin need to worked on.

You probably have what people call a baby face.

Will I get the lefort for free?

Nope, unless your country has a national health service or you have insurance. I wouldn't count on it as these are for functional issues.

All maxilo facial surgeons do lefort 1 . You will probably need the bsso , genioplasty or imdo.

For lefort 1 it's like €6000 in Europe.
