From what I know Sean O Pry has done otoplasty (changing monkeys ears to normal ones). Modelling agencies may do surgeries but Barrett is an exception because he is crazy

Otoplasty isn’t a very invasive surgery, wouldn’t even consider it hard maxxing

No model/actor in their prime gets surgery because the risk doesn't outweigh the reward. You can see this with the recent buccal fat removal trend. Different story when they get older and start losing hair, wrinkles, saggy skin etc
