Facial fat pads are the pockets of fat that give the face its volume and youthful appearance. Over time, as we age, these fat pads can shrink or shift, leading to changes in facial contours (such as sagging or hollowing). Tretinoin doesn’t directly affect the fat pads themselves, but here are a few considerations:

  1. Skin Thinning: Tretinoin can increase collagen production and thin the outermost layer of skin (epidermis), making the skin more delicate and prone to sensitivity. While this thinning mostly affects the skin's surface, it can give the appearance of less youthful fullness in the face, particularly if there's already a natural loss of facial fat due to aging.
  2. Volume Loss Over Time: If you're prone to volume loss in your face due to age or genetics, the enhanced skin turnover from tretinoin could make any natural facial fat loss more noticeable in the short term. However, this is a cosmetic concern, not a direct result of the medication.

In Conclusion:

Tretinoin is not typically considered harmful to facial fat pads, but in some cases, the changes it induces in skin texture and appearance (like thinning or dryness) may highlight pre-existing volume loss. If you're worried about this, maintaining proper hydration, using a good moisturizer, and possibly balancing your routine with other products that boost facial volume (like facial oils or peptides) can help counteract these effects. If you're particularly concerned about fat loss or changes to facial volume, consulting a dermatologist might be helpful.
