I have read somewhere on internet that jaw surgery can relapse over time. Is it that common? My idea was going to Ramieri but I know he is pretty conservative. Do I risk to spend 15 k for nothing and to find myself at the same starting point? Both my jaws are recessed

He's not particularly conservative. That's something that got started because his results weren't overtly forward grown like Sailors ba's

Well it doesn't move maxilla over 4 mm

Jaw surgery *will* relapse in the period of 1 year. The question is by how much. In most cases it’s not noticeable. If the surgeon fucks up or you are unlucky as hell, then yes. After 1 year, relapse is literally impossible unless you have a very weird disease of which I don’t recall the name.
 
Do custom plates make relapse less likely? Are they more robust?

Relapse doesn't mean that your jaw returns all the way to its original position. It just means that the final result is not as big of a movement than was planned. So if you get 10mm advancement and it decreases 2-3mm from that, that's considered a relapse.

I've never heard of a 100% relapse if the original jaw surgery movement was successful.
 
if you get rotation and also asymmetries corrected p sure that s bullshit.

For instance Ramieri moves 4 mm at maximum for Maxilla. Would it return to the original? My main concern are both jaws

I would guess the answer is no. I've never heard of a 100% relapse from a successful jaw surgery. I'm not a doctor, though.
