Hey, I made 2 previous posts about my journey to jaw ascension in case you're interested in the whole pre-operative treatment. But 6th of August is the date.

Short background: I'm 20, living in West-Europe. Wore braces when I was younger to fix my overbite but it came back to a class II malocclusion of around 6mm. Started pre-operative Invisalign treatment in November.

I went for the 3D scans and virtual planning last week (so I have more detailed measurements too: overbite of 4mm, overjet of 8mm). The plan is to get a lower jaw advancement (BSSO) and removal of 2 wisdom teeth, depending on the scans they made they will decide if I need an additional upper jaw advancement (bimax) or genioplasty.
I'm feeling pretty nervous about it since I've never had any surgery done so I'm unsure how I'll handle the pain. Also biggest catch of em all: my preferred surgeon won't be doing my surgery because he broke his dominant arm... He'll be there during surgery to guide a surgical resident. I trust both, but it is a little unexpected.

you’re really gonna let a resident do trimax on your face? You realize these people literally put like angle grinders on your face to cut the bones open right?

If you’re paying with your own money don’e let a fucking resident perform on you brah , also , you said they might do genio , bro insist on it it should be a given , anyways please consider doing a post op thread too i’d like to follow your journey

This seems sketch asf.

If they’re letting a resident do it then it should be either free or discounted.

U probably do need a genio, you decide it for yourself, not them.

so far it's not trimax. most likely just lower jaw surgery. when i saw the surgeon (with the broken arm) last week he told me the resident had been doing 10 surgeries this way since his arm broke. i don't think the surgeon would let him do it if the resident was unskilled

i'm paying around 1000 euros myself. the rest is covered by health insurance. i'm seeing my surgeon on the 5th of August to talk the plan through based on the measurements, photos and scans. i'll make sure to tell him that i'd like a genio then. and also ask some extra info on the resident. i'll make a post op thread too. i'll have plenty of down time in my bed to write here lol
why would you consider a genio obligatory without knowing my anatomy? is there something i'm missing?

i doubt it's sketchy. it's a public hospital so i assume the resident is capable of performing the surgery if he gets a green flag from the surgeon. but prices are standardised along all public hospitals here so there's no such thing as a discount
why do you think i'd need a genio? i'll make sure to mention it when i see my surgeon on the 5th of August to talk through the plan

Not “sketchy” as in its going to be botched, but sketchy because you are paying thousands to probably get under advanced, and be disappointed with the results.

Almost everyone who gets a bimax also needs a genio of some sort. We can’t see your face, so can’t make the judgement but I would say 99% of the time a genio is needed.

i'm paying around 1k (not including orthodontic work and post-operative care), the rest is covered by health insurance. how much advancement will be made is up to the original surgeon (the one with the broken arm). he's making the plan and the advancement will be made following a 3d printed splint.

so far i'm not getting a bimax, but a "simple" lower jaw surgery. surgeon told me that the in the majoity of cases this is enough. but depending on my scan results, an upper jaw surgery and/or genio can be added, but only a small percent of patients benefit from that. personally i think i need a genio too. there's a lot wrong with my chin (mentolabial angle of only 62° and mentalis habit) that i think won't be fixed by a lower jaw advancement

thanks for being critical though, that's why i come on here

Almost anybody can benefit from a genio , and it’s the most important part of the trimax aesthetically speaking

omw right now. they showed me 2 options yesterday: lower jaw advancement with or without genioplasty (2mm)
not too much difference cuz 2mm isn’t that significant ofc, but i did choose the option with the genioplasty unless someone wants to talk me out of it now

2 mm won't make a visible difference anyway, i'd still get it just to stretch skin a bit

wasnt big of a recession then
not gigantic, bit surely noticeable in the photos.
i forgot the numbers of advancement for my jaw. i just have a paper from the measurements a few weeks ago that says i have a 4mm overbite and 8mm overjet.

for chin they did say 2-4mm with rotation but don’t know what they ended up going for

they did 7mm on the jaw and 2mm on the chin

no complications, i’m home!
sounds good, why the rotation for the chin? did u lack chin height?

small rotation so it would line up with the bottom of the rest of the jaw bone. so more practical

Nice bro

Just found out I got sleep apnea lol.

Gonna get trimax with implants

I want to do a medium sized movement, because I’m not death tier recessed.

Perhaps 4mm upper
6mm lower
5mm chin

Yeah Valerio Ramieri, very pricey but his results are mogger.

I’ve heard French surgeons are cheap and are pretty good, but I don’t know any tbh.

that was very much out of my budget. are you french? i did my surgery in a country neighbouring france for around 1k-1.5k (depending on what health insurance will cover)

Nah I’m from UK, the worst and most expensive surgeons in the world lol.

Which country? PM if u want

I got trimax coming around February 2026 with dr. Kettoff in Paris.
Social security takes 2k3, my insurance takes 2k3 and I pay 2k3 euros.
His got pretty good results I think, but I don’t know yet the distance of movement he wants to do on my jaws. So maybe he’s bluepilled idk

go for bimax anyways, 99% of people would benefit from SOME lefort movement

also italy (except for pagnoni) or death

u're a lil late

most swelling has gone down. ofc this is nowhere near the final result but i’m really happy with how my lower face looks. my facial harmony is off however. never had a problem with my midface until now. it just looks so flat :(
