It's actually worse because increasing intelligence is an inherently more difficult problem than improving your appearance. For the latter, we have softmaxxing and surgeries though they are still risky and not as effective as we hope. But I can see your appearance becoming something you more or less choose in this century. Increasing your IQ or even changing core aspects of your personality are much much much further away because that would require big breakthroughs in understanding how the brain works.

I wouldn't necessarily say we understand it that much better than 20 years. Sure, we have AI that can do some pretty impressive morphs but they look weird/uncanny 90% of the time. What's still lacking is our understanding of 'harmony'. I think we will see breakthroughs in this specific area but it will be AI driven. I'd give it 20 years or so before we can morph someone into the most ideal version of themselves that still looks convincing and realistic. And then another 60 years before we achieve some form of morphological freedom. But imo, I think if we have AR based therapies, we can cure most forms of 'BDD' or whatever it is called in the near future. No need for invasive surgeries except in a few cases where therapy isn't effective.
