How much does lower eyelid retraction ascend? Is it uncanny? How risky is it?

Depends on how bad of a case it is, Ive seen some good results online from it and some very mediocre. But it's often combined with other procedures I think, one procedure alone won't ascend most people. Don't know how risky it is

Good to note. I’ve been thinking about combining it with fat grafting.

It will look uncanny if there's an overcorrection (make you look like youre squinting permanently). But the lid being too high is not as likely, more likely the lid will relapse and come back too low. This is likely if you have a negative orbital vector which is most often the reason for lower lid sag in the first place. So you'd need that fixed alongside, for stable lid results.

The surgery is not high risk for your health, but rather for your looks. Doing any invasive work on your eyes is risking your looks and harmony and should only be considered if someone has seriously chopped eyes. Fat grafting should be prioritized instead. Just look at Frank Tufano, he is a Frankenstein project at this point, and the doctor fucked up his lower eyelid retraction.

From what i can recall, he was mainly botched for getting orbital decompression. I’m planning to get fat grafting as well but I do have small but noticeable sclera show and rounded eyes

How do you fix the negative orbital vector? Implants or implant w/ saddle?

Either you have to move the eye back (orbital decompression) or you have to move the area under it forward (filler, fat graft, implant, lf3), sometimes both. Implant will improve vector by itself without saddle, saddle purpose is to smooth the transition to the lid.

And which one of these is indicated varies greatly between cases (except lf3 which no one is getting).

I want to get fat grafting w/ the retraction. That would work with minimal risk of relapse right?

Depends completely on how bad your vector is and the cause. If you have proptosis (bulging eyes) it won't be enough. If you have decent enough midface bones and your eyes aren't abnormally large and shallow set then fat may suffice. But people like that usually don't have lower lid retraction anyway, at least not when young.

This is my eye area

Overall you don't look bad at all for an Indian. If you really want to go all in, lower lid retraction and then fat grafting can do the trick. If you want to take it easy and safe then fat grafting alone can also improve your eye area by alot, by filling out the lacking soft tissue.

I mean you do lack infra fat (almost all Indians do). Need to see side profile to judge vector properly though. But the lower lid doesn't look that bad in this pic.

Thank you. I’m just worried that if I just fat graft the results will be underwhelming compared to getting both procedures. When I smile, the lower eyelid becomes a lot more prominent too.

Do I have a negative orbital vector?
not at all
looks positive but your head is tilted up, correcting for that perhaps neutral. you just need tear trough fat to smooth that transition from lid to cheek

The sclera is barely even visible in all of your pictures despite your sunken lower eyelids. Fat grafting will push up the lower eyelids slightly, so it helps with that too. Saddled infraorbital rim implants might also boost your looks by a lot, but they are very expensive and it's hard to find a good doctor. The implants also slightly push up the lower eyelids.

I’ve heard about implants with saddle but yeah there are only like 2 docs in the world that do that and can’t find any results online. I didn’t know that fat grafting can also push lower eyelids up a bit.

It won't raise the lid meaningfully. Even implants don't, and even with saddles don't always.

And you don't need implants, would be a misuse and may look uncanny. Get infra fat (and supra if you wish) and then you probably don't need anything else

This is the most realistic looksmaxing scenario. Surgeons might not even take him as a patient because the lower eyelids aren't even that bad.

I did exactly that i got lower eyelid retraction repair plus fat grafts

You do, fat grafts wont permanently raise your lower eyelid my vector is positive (have a dvt scan aswell) still fat grafts for shit fat pads are a yes, lower eyelid retraction ist often because off the muscle so its need to be correctws
