Don't know which lower third/jaw surgery is best for you? This guide should help. I complied all the potential surgeries with pros and cons to help you decide. This should cover everything, lmk if I missed anything (TLDR at the end).
1. If Your Chin is Recessed (Weak Chin)
Sliding Genioplasty – Moves the chin bone forward.Chin Implant – A silicone or Medpor implant placed over the chin bone.Best for: Weak chin but normal jaw.
Pros: Permanent, natural-looking, no implant needed.
Cons: Requires bone cutting, risk of nerve damage.
BSSO (Bilateral Sagittal Split Osteotomy) Advancement – Moves the entire lower jaw forward.Best for: People who don’t want bone surgery and need mild to moderate projection.
Pros: Quick procedure, minimal downtime.
Cons: Can look unnatural, risk of implant shifting, less control over shape than genioplasty.
Best for: If the whole jaw is recessed, not just the chin.
Pros: Improves bite and aesthetics.
Cons: More invasive, requires braces pre/post-surgery.
2. If Your Chin is Too Long (Prominent/Overprojected)
Sliding Genioplasty (Setback/Reduction) – Moves the chin bone backward or reduces height.Chin Bone Shaving/Reduction – Directly shaves off excess bone.Best for: Chin that sticks out too much but normal jaw.
Pros: Permanent and precise.
Cons: Requires surgery and bone healing.
Bimax (Bimaxillary Surgery with Mandible Setback) – Moves the lower jaw back.Best for: People who only need a small reduction.
Pros: Less invasive than genioplasty.
Cons: Can’t move the chin back much, risk of bone asymmetry.
Best for: If the whole lower jaw is too large.
Pros: Fixes overbite and improves bite function.
Cons: Major surgery, requires orthodontics.
3. If Your Jaw is Recessed (Weak Jawline)
BSSO Advancement – Moves the lower jaw forward.Jaw Implants (Mandibular Implants) – Silicone or Medpor implants along the jawline.Best for: Recessed lower jaw causing weak jawline.
Pros: Improves bite and aesthetics.
Cons: Requires braces.
Custom Jaw Augmentation (PEEK, 3D-Printed Titanium, Medpor Implants) – Custom-fitted implants for precise jaw enhancement.Best for: Aesthetically enhancing jawline without moving bones.
Pros: Quick and effective.
Cons: Risk of implant movement, infection.
Best for: Severe jaw deficiencies.
Pros: Tailored for facial harmony.
Cons: Expensive, requires skilled surgeon.
4. If You Have a Strong Jaw but Poor Definition (Lack of Sharpness)
Jawline Liposuction – Removes excess fat under the chin and jawline.Buccal Fat Removal + Jaw Implants – Reduces midface fullness while adding structure to the lower third.Best for: People with soft tissue covering their jaw.
Pros: Minimally invasive, quick recovery.
Cons: Won’t work for weak bone structure.
Masseter Botox – Reduces jaw muscle bulk for a more V-shaped face.Best for: Chubby faces that lack definition.
Pros: Can create a chiseled look.
Cons: Buccal fat removal can prematurely age the face.
Best for: People with overdeveloped masseters (not bone-related).
Pros: Non-surgical.
Cons: Temporary (lasts 3–6 months).
5. If Your Jaw is Too Wide
Jaw Reduction (Mandibular Angle Reduction Surgery) – Shaves off part of the jawbone to narrow the lower face.Masseter Botox – Shrinks the jaw muscles.Best for: Naturally wide or square jaws.
Pros: Permanent reshaping.
Cons: Risk of over-reduction leading to weak-looking jaw.
Best for: Jaw width caused by large muscles, not bone.
Pros: Non-invasive, no downtime.
Cons: Temporary, needs redoing every few months.
6. If You Have Facial Asymmetry (One Side Looks Different)
Asymmetric Genioplasty – Adjusts the chin to align with facial symmetry.Asymmetric Jaw Surgery (BSSO One-Sided Advancement/Setback) – Moves one side of the jaw differently from the other.
Facial Fat Grafting – Adds volume to the weaker side.
Masseter Botox (One Side Only) – Shrinks a larger masseter muscle for balance.
7. If You Have a Long Midface (Excessive Vertical Growth)
LeFort 1 Maxillary Impaction – Moves the upper jaw higher to shorten the lower third.Sliding Genioplasty (Vertical Reduction) – Reduces chin height.Best for: Long midface + gummy smile.
Pros: Creates better proportions.
Cons: Major surgery.
Best for: Long chin, but normal upper face.
Pros: Fixes lower face imbalance.
8. If You Have a Receded Midface + Lower Third Issues
Bimaxillary Advancement (Bimax Surgery) – Moves both upper and lower jaws forward.Maxillary Advancement + Genioplasty – Moves upper jaw forward while also enhancing the chin.Best for: Flat midface + weak jaw.
Pros: Full facial balance.
Cons: Big surgery, requires braces.
Best for: Midface deficiency + small chin.
