Would a fat transfer emulate a higher cheekbone and better midface support?
I would get malar implant but I want to avoid bone erosion. Is fat transfer a better idea?
What do you think?

Nope, implant only.

Bone erosion is minimal on zygo/infraorbitals because you don't use the muscles that much vs like a chin/jaw implant... you use your jaw when speaking/eating basically all day. Keep in mind though you will have some bone erosion over time regardless.

Implants for sure

Just don't go overboard with it, I got 5mm projection on mine and another surgeon said that is the max most guys should get.

Regardless I need that, my under eye hollowness is not good.

Fat is grafted and then injected into the subcutaneous levels of the skin, not onto the periosteum (bone layer).

It is not effective in producing a high cheekbone aesthetic, since this trait is apparent by having low levels of subcutaneous fat juxtaposed against a prominent zygomatic bone.

Malar implants are used to increase the projection of the zygomatic bone. Adding fat here will only further conceal it.

Fat-grafting is effective when used to augment the front medial cheek, in the deep malar fat pad.
