im a 16 year old male. Current height: 5'11.3, Bideltoid: 19.1 inches. making this post almost as a log to share my entire plan and get help on what i can do to improve it/revise it. i am planning on posting my entire journey on here.

i have a slight structureal underbite not too drastic and got invisalign to try to fix it at around 14.5 years old. huge waste of time + money. Recently came to the realization I need to stop coping and get bimax to fix it. I visited a bimaxillaryfacial surgeon around 2 weeks ago; they approved for for MARPE, although I didn't have any breathing issues and my pallate isnt rly narrow but the surgeon was pretty open to it and conviced my dad to talk to him and approve it.
Goals:

  • Maximize natural growth before sutures fuse
  • Widen and advance midface
  • Sharpen jawline + chin
  • Set up ideal structure before adult surgeries



⚒Phase 1: Development Phase (Age 16–17.5)​


1. MARPE + Facemask Protraction

  • Method: Bone-borne MARPE + Delaire facemask Will do both of these for approx 6 months (while running hgh + igf1)
  • braces after this for 8-12 months to fix teeth allignment before bimax

  • Trying to get good midface base without surgery early

2. GH + IGF-1 Protocol

  • GH: Start of with 3iu a day for first 2 weeks, 4 for the next 2, and 6iu for the next 8-12 months. I will start it as soon as i get my MARPE done.
  • IGF-1 LR3: 30–60 mcg/day or 3x/week

  • Goal: Stimulate bone growth, widen zygos, increase ramus height, thicken mandible
  • Timeline: 6–12 months depending on progress


    I'm unsure on how much dosing of IGF-1 LR3 and IGF-1 DES I should be taking would appreciate advice on that.

It won't cause maxillary projection. There's a reason why orthodontists only use Facemasks for kids under the age of 9-12, beyond that point they simply don't work because some of your sutures have already closed.

I'm sorry but no. It's like a palate expander, in truth a palate expander only works between the ages of 14 to 16 or under, beyond 16 the palatal sutures have already closed, meaning that they've become hard bone, so no amount of expanding will separate them anymore.

On which case an MSE device is used, you will never get a palate expander as an adult because it's simply useless.

Same case here with a protraction device, in this case the maxillary sutures closed even earlier, and after they've been closed no amount of will cause any forward growth. That's like trying to pull a rock and expecting it to become longer. Even with HGH, if the sutures have already been fused, you will not get any forward growth from this.

Don't trust me, trust the orthodontists who utilize this technique, they'll explicitly tell you that beyond 12 (or really 9 on average) this is just useless for you.

Okay maybe my language wasn't clear, they ARE palatal expander in the outcome they provide, but a traditional palate expander is only used in children while MSE/MARPE are used in adults to expand the maxilla, I was speaking of the devices not the outcome specifically. Anyways, it was meant to be an example of how once the sutures close certain stuff doesn't work anymore

havent done facemask, not doing it. got marpe becuase its more structural and the doctor reccomended it. to answer if it was worth it, and if i shouldve rather gotten bimax/ccw initially obv surgery is better but i am already schduled to get bimax( insurance is likely planning it) and genio w ccw (out of pocket). even if the bimax isnt covered by insurance i plan on paying out of pocket. i just ordered by HGH, its ordered and arrives in a week ill start pinning then and keep u updated (also pinning test w it). 16 might be a good age for marpe, most orthos dont rly do facemasks at that age and MARPE is whats reccomended (or mse or sum invisalign expander).

Wrong. You can use facemasks if ortho is using MARPE. After midpalatary sutures are split (diastema is visible) there is a ~1 month period where maxillary protraction can be possible if patient wears facemask at least 14 hours a day (im including sleep time) + is compliant. If compliant, 4-5mm can be achieved. Stop spreading information about shit you dont know. My sources are Dr Lipkin and Ting

money isnt really an issue, ive smoken to a maxillofacial surgeon and might try to get insurance to approve if not was planning on doing the surgery in turkey or india. why CA tho? if theres some real benefit I might be able to

The sutures can be separated with enough force, expansion is completely possible in adults.
