It's not that their head is small, it's that their jaw is recessed.

The key to having a good lower third is good forward growth, there must be a huge slope between your chin and your forehead, that is how you get a good lower third like mine. Most people aren't born with it though but a maxillofacial double jaw surgery can give you that look.

In my case, I always had lots of forward growth, but too much because my lower teeth would clench in front of my upper teeth, like Dicaprio's wolf impression in Wolf of Wallstreet (or @FastBananaCEO's avi), my teeth were stuck in that position for most of my life which made me look like an ogre/caveman.

In 2018 (when I was 20) I had a maxillofacial jaw surgery done to fix it and now I look great, my forward growth is superior to the vast majority of the population even Chads, because my jaw and my forward growth are very big, but my teeth now rest in the right position, so I don't look like an ogre or caveman anymore. If I were to add jaw implants in the back of my jawline, my jawline would be extremely impressive and it would mog the one of most male models even.

So, having a big head is important but having tons of forward growth is even more important.
