Cant get a real answer from literally anyone. Does mse with surgical assist still increase the nose base or only affect mouth width. Is the cut a full cut or what. I dont want to do retard's chewing method and not get ipd gain.

I had surgically assisted MSE done 4 weeks ago. To answer your question, yes my nose AND my IPD both got noticeably wider. Not enough for someone you know to realize unless you point it out to them. I would guess around 1.5 mm increase. The whole reason I spent 3.5k on surgical assist was to avoid IPD increase since my eyes are already wide (I even paid photoshop surgeon beforehand to see what I'd look like with IPD increase). So that was disappointing, but it's not **that** bad

My situation is complicated though. Here's a breakdown. I'm a male in my mid 20s. When I went for surgical assist the surgeon told me my bones were very hard and he had to do 14 turns in order for me to get a diastema. I thought to myself WTF. Right after surgery I notice that my nose and eyes both appear wider and that my face is swollen like a chimpanzee, but I just thought it might be due to the swelling. He instructs me to do 2 turns a day after that instead of the standard 1 turn a day protocol.

So I do 2 turns a day and after about 5 days I realize that my diastema isn't getting any wider. I saw the surgeon again and he told me it's fine, just continue making more turns. So I continue making turns for another 9 days and the diastema still hasn't gotten any wider. At this point I think the MSE screws are just dragging through the bone or the appliance itself is bending. This following monday I will go to the orthodontist and ask him if he can make additional cortipunctures to weaken the suture.

But anyway to answer the question about the nose and eye width. My swelling is 90% gone and my nose and eyes both are wider. It's a huge falio but not noticeable to a point where people I know personally would comment on it.

To make matters worse, my MSE wrench got stripped while I was trying to make a turn so it's no longer capable of turning the screw. Now I'm waiting for a new one I ordered to come in the mail.

Are you 100% sure abt those changes? ive seen palate expansion via mse b/a and their eyes show no difference, plus 1.5 mm is a giant amt. i was told that the smarpe almost completely detaches the lower region and upper face differences would be unnoticeable, especially not in the orbits.

Cheekbones expanded without question.

The cut by Vaughn (OMS that Ting uses for surgical assist) is not all the way through the bone.

Other OMFS might do it differently.

Yes it definitely widens the nose still that is the most noticeable outward change

I went to Dr Vaughan for the surgery. The lefort cuts do not go all the way through the bone. Vaughan told me he did monocortical cuts in the front and bicortical cuts on the side. Moreover the cuts do not go all the way back to the molars, only the bicuspids.

I'm not comfortable sharing pics at the moment (I am ugly AF, everyone on this site mogs me). Interestingly the eye width changes appeared the exact day I had the surgery, where vaughan split open the mid palatal suture with a chisel and did 14 turns on the MSE. Even though the swelling has gone down, the nostril width and eye width did not change. I am confident both of them increased as I look different, but I don't have any measurements or anything to prove it with.

Also, I've been doing 2 turns a day and my diastema has not been getting wider, and it looks like the MSE is literally bending/buckling. It's as though I am failing to expand even with surgical assist. On Monday I will see Ting to make sure that's what's happening. Anyway, the eye/nostril width thing hasn't gotten worse after the surgery, so that's a plus.

you should buy a digital caliper to keep track of those differences if u truly consider them significant. it just seems weird to me bc ive seen many b/a and noticed almost zero facial change, especially not to minor parts of the eyes. by eye width, do u mean inner to outer canthus or pupil to pupil? if ur willing to, u could send pics with with only your two eyes showing with everything else blacked out, and i could make measurements abt the changes using the ratios to the iris (which would tell u if its due to some weird soft tissue response or actually different).

idk man, like u see certain changes on the scans, but in actuality those slight changes are virtually not there. i guess it is just a palate expander meant to open up mouthbreather airways, so facial changes are much to expect. cant rly tell until u have in ur mouth and ur looking in the mirror tbh

Man im a nosebreather I got this shit for facial changes. Guess I just burned 10k man.

i havent had a non aching throat in the morning in forever and am a heavy mouthbreather, so alleviating that would be enough for me. just dont wanna fuck up my face while at it tho jfl

I had a CBCT taken before the surgery and I will take another one after my expansion is complete. That will give me a definitive answer how much the pupil to pupil distance has changed.

@RealSurgerymax i got a visit soon for an MSE I don’t want my eye socket to spread apart. I like the distance between my eyes.
Should I reconsider it?
 
Yes. But that can be fixed surgically.
Do u have a diagram of what changes and the severity of the change. Also does Mse affect the medinal canthus since the nose base is also changing. Thanks.

Are you sure about the IPD ? Im reading and it definetely not changes the IPD
