Hi all. I think it is my duty to update the forum on how my surgery went.

I joined .org exactly three years ago and I now feel partially indebted to you all for the learnings. As most of you, I came in here looking for answers, which eventually brought me to explore a deep rabbit hole. This caused me to opt for corticotomies, MSE, aligners, veneers, and now, trimax and infraorbital implants, which I will be describing below. Quick teaser on the results (different angle and all that but you get the idea):



Starting point
I departed from an already forward grown maxilla but a noticeably recessed chin, with a long and narrow face and bad under eye support.

Plan
- 2 mm maxilla advancement
- 2 mm maxilla anterior impaction
- 2 mm bsso advancement
- 1.5 mm ramus widening on both sides
- 9.5 mm genioplasty
- 5 degrees ccw
- 2 mm asymmetry correction
- 3.7 mm max in the infraorbitals

Outcome
So far everything has gone according to the plan. It has resulted in a remarkable 15 mm menton advancement, which has given me a nice anteface, hence I no longer look recessed. The ramus widening came in handy, making my jawline more visible, and my eyes look fresher thanks to the infraorbitals. It is still too early to judge some aspects given the residual swelling, yet all in all my expectations were met.

What’s your new bizyg/FWH, bigonial, and gonial angle post-op? Also curious how much the ccw + genioplasty changed your cervicomental angle and NHP profile.

Have you noticed any visible results or changes of improvements on how youre treated?

Not much in terms of aesthetics, just pushed my zygomas a little bit towards the side. It mostly served to make room for the teeth and reach a nice bite position that then allowed me to be a surgery first patient. Of course having a wider arch is an advantage, despite it only being visible when I smile

Alrr cus I need it 100% good to know it helps elsewhere too

The bizygomatic width stayed the same as the infraorbitals did not give me lateral projection. They were merely to improve my under eyes. As for the bigonial, about 3 mm of increase, minor but noticeable. My face width to height ratio has remained the same pretty much. I would estimate the gonial angle has been reduced by close to 10 degrees, now approaching 125 degrees. There is barely any difference in the cervicomental angle as my bsso was minor and most of the results come from genioplasty. As for the Nottingham Health Profile, I am just a few weeks out of surgery so it is complicated to reach accurate conclusions for now; but I would say I had a good life to begin with, no pain, spend everyday doing what enjoy, so maybe just a couple points will improve.

Have you noticed any visible results or changes of improvements on how youre treated?
Too early to tell as I am still swollen, had a black eye for weeks, and was generally treated well to begin with. But so far I got some "it was a pleasure to meet you" and appeared funnier.

how many mm expansion ? and did it widen ur lips ?
 
I wpuld literally just suggest lips

Nice result. What surgeon placed your implants? Is there any minor visibility of the edges of them when you're in bright lighting conditions like a sunny day?
 
how many mm expansion ? and did it widen ur lips ?
About 8.5 mm, although I seemed to relapse a little bit. The lips stayed pretty much the same. What changed the most was the bite improved, reducing the lip incompetence. I have always had full lips though.
 
Nice result. What surgeon placed your implants? Is there any minor visibility of the edges of them when you're in bright lighting conditions like a sunny day?
The one and only, Dr. P., how may just be the best surgeon worldwide for implants. I have not noticed implants being visible from the outside, no matter the angle or lighting. They are very subtle, do not project more than they have to, and nobody in my circle has been able to point out I had implants.

Is mse even worth it for aesthetics? Be honest did u ascend or descend even if just slightly in one way. Almost everyone descends from what i see.

Also your genio is way too much, you shouldve gotten more bimax advancement if you needed such a huge genio to have your chin look normal

How were implants? Good and not uncanny? Who designed and placed?

Not descension from MSE. It helped me improve my bite, which had an indirect positive effect on aesthetics. Also a bit better zygomas. In general, MSE is hardly going to ascend anyone. It had a neutral/slightly positive effect on me.

Would you say my chin looks abnormal now?



The implants are alright. The design was conservative to begin with, so I was not expecting dramatic changes there. It helped me get rid of very depressed under eyes and have a fresher look. I am still a bit swollen on the cheeks area to fully appreciate the outcome from implants.

My surgeon was Dr. P. and I had a very positive experience with him.

It looks like the genio may have been overprojected. You started with a very weak looking jaw/chin so i think you couldve done more bsso instead, and prob more le fort 1 as well, but oh well better to be more conservative than overproject your face i guess especially if u didnt have sleep issues….
You should maybe consider get your genio slightly reversed if it projects past your lips at all i think….
