When you’re born with recessed cheekbones :feelsrope:

Your soft tissues; malar fat pads, ligaments etc. will sit lower on your face as a consequence of structure but also the fact that gravity acts harsher on the fat pads as they have little to no bone support due to the recession

I got malar/zygo implants but what I didn’t think about was the fact that my soft tissues have had 20 plus years of sitting in the region under my cheekbones, above the paranasal fold

Like this:

So when I got the implants placed at my zygo level, of course they didn’t help raise these fat pads as the fat pads are below them :feelscry:

So what to do?
Answer: deep plane mid face lift

A DPMFL raises the entire mid face soft tissue as one unit and places it higher in the mid face like so:

The beauty about facelifts is that the ligaments are cut and the fat is held in place by sutures attached to the temples moving forward.

So that even if you don’t have good cheekbone support, it doesn’t matter because the primary thing holding them in place is the sutures.

A little fat grafting to smooth things out and you now have a mogger midface; better ogee curve, reduced nasolabial folds, less undereye bags etc.

RISKS
Now doing a midface lift in your 20s/30s does carry more risk, this is primarily for 2 reasons.

Firstly, any lift risks nerve damage as your cutting and moving so much of your face.

But secondly, any lift makes subsequent lifts harder, so if you get one now, the next one you get in your 40s/50s will carry more risk.

If you only care about looking good now it’s okay, or you can accept that risk and plan to get another 1/2 lifts in your lifetime like I do.
