Need advice about surgery

I need some advice about what, if anything, I should do to my face. Surgery? Appliance?
My eye region is pretty good. Jaw / chin sucks. Forward head posture is getting better (somehow it was worse than this before). I chin tuck a lot.
22 yrs old
6'3
190 lbs
I've also attached some pictures with a beard because I was blessed with a major cope called a good beard. It covers up some of the recessed lower third look, but I'd rather have a face too.
I mew/chin tuck and lift.

I wanna suggest a bimax but your upper maxilla is very recessed too. unironically you need lefort 3.

A chin wing would do it. Bimax would be marginal improvement because you don't have any significant deformity or major recession imo. Go see Zarrinbal (i've consulted with him) he's very good.

Zarrinbal is in germany right? You don't see recession? I see huge recession am I missing something.
jaw implant would ascend u hard, but ur base is pretty good either way.
Implant over bimax? Why do u think this?

Yes, in Germany. I see recession, but nobody can tell you how much bimax will truly help without ceph analysis or knowing what the occlusion looks like. Many cases similar to yours from a cosmetic sense are often average. A chin wing will give you further projection of the mandible and give some drop down (which I think would help asethetically). Zarrinbal can do some additional non bimax work on your upper jaw (which I will not detail on here) if you combine with a rhinoplasty.

Ultimately, your best bet is to see a handful of maxfacs and ask. If I was in your position personally, I would get a chin wing and then reassess after and I say that as someone that underwent bimax. You can get bimax after a chin wing but the reverse isn't always true due to the nature of a BSSO cut. Most of the chin wing docs will also offer bimax + chin wing - that's another route to look at.

Edit: Also your midface appears somewhat flat (I don't see much anterior cheek projection). If you get maxillary advancement, the midface isn't really touched, but often times can appear flat. So, another thing to consider.

Okay those are some good pieces of advice. I think your point about getting a chin wing is definitely true because my chin is pretty recessed. Also seeing multiple docs.

Chin wing doesn't only impact the chin. It's a cut along the mandibular border, so you're impacting the lower jaw. You can get drop down as well, which doesn't happen in isolated chin movements. It's a pretty good surgery from an aesthetic pov as long as the jaw angles don't get blunted. And Z's work is pretty damn good in this regard.

If you opt for bimax, see someone that is aesthetically minded (a quick google search will yield good surgeons to see). You'd probably be looking at some advancement with a posterior downgraft and more often than not, local surgeons don't really want to perform rotation or can't. Many will also do some sort of cheek grafting whether that be hip, cadaver, fat, filler, or hydroxyapetite to counteract midface setback.

My g it looks like you are balding imma be totally honest you look alright atm even with a recessed face but once you lose that hair it’s over beyond imagination

Pros and cons to both, not going to list them extensively because everyone has their own bias towards procedures. Bimax is much more invasive, would cost more, and yields more changes. Chin wing is obviously less costly and intensive because you're only impacting the lower jaw and not touching the bite at all. There's obviously good results for either one or both together. My recommendation is to read up and form your own opinions after speaking to more than just Dr.Z. In the end it's about what you perceive as the risk/reward that will determine what you end up doing.

I'm not against bimax. I'm just saying I doubt it'll change your profile as much as you think. Jaw implants are also a good call because you can get angularity, width, projection that no osteotomy can provide. Granted, bone movements will help with better implant results, imo.

Facts lol I've been fighting balding since I was 16 xD Used fin and minox for 5 years now exosomes because i got bad sides from minox.

Yeah that's good advice.
I thought implants only last like 20-30 years though. I'm 22 rip.
