Need Eppley Referral Card to get CT scan done

Hey everyone

I got a genio done and it was botched looks really bad.

I need a revision but have to get a new CT scan to make new plates to move the chin.

However because the surgeon is out of country they won't take referrals from him so I need to do what I did last time and fraud an Eppley Referral, but I lost the referral card that people used from Eppley.

Anyone know what to do? Getting a CT scan is so easy but they make it difficult with the referrals.

Wtf is this black market knowledge I did not know of ?

Is there a generic Eppley referral that imaging centers are accepting to get scans done?

You can contact online referral radiology services lol- they sell the CT scan with referral after you answer a few generic questions
