Need help with my upcoming Double Jaw Surgery

(basically a repost of the thread I did on the wrong subforum but with extra questions)

I already have a Lefort I DJS scheduled for next week, and just received the planning from my doctor. While it's going to be covered by insurance due to sleep apnea, the doctor gave me some leeway to solve some aesthetic concerns.

Besides the obvious short chin, one of them is my bimaxillary protrusion:

That being considered, here's what my doctor proposed:

One of the reasons I'm posting this is because the doctor also gave a preview of the simulation on my first consult. And honestly, it kinda looked better imo, but I could be wrong:

Given that, I'd appreciate any help since that a once in lifetime chance do hardmaxx. My main questions are:

  • Should I go with his planning, or the preview? (He went more conservative on the planning itself cause he's waiting for my insight, so don't take that as he's final suggestion)
  • Should I rotate or push the jaw or chin even more?
  • Does rotating too much risk increasing my nasolabial fold, or making my mouth area even more protruded?
  • Is my lower third too short to rotate like in the preview, or more than that? Should I maybe ask him to not rotate that much and then just push it further?

Small movement but a good one, anything else op?, like jaw widening or 2/3 piece le fort or just the movements you are getting?

But be sure to run through what the exact movements will be and how they differ with the initial preview.
Never act on an information vacuum.
Superficially, both plans look good; I would just go with your personal preference.

Just these movements, really. I don't think he would do anything much more complex since it's mostly a health related surgery. I'm thinking about midface and ramus implants after, but I want to see my results first.

Honestly I've been getting kinda worried about what to suggest. I've received mostly positive feedback from people here, saying the movements are fine. Meanwhile on the Jaw Surgery subreddit, strictly "no looksmaxxing" they're all saying it's not enough advancement, too much genio, still recessed etc lol

You don't always need to have a strong ante face to get a good result actually, this can get you dogmaxxed and ruin your face more, since your lips and chin are in line, it is the best thing you can actually get.

If your preference is to have a somewhat strong ante face then go for it, but you'll also need to take into consideration the facial depth and the frontal bone.

But your plan is good.

Well your surgical plan is also meant to address bimaxillary protrusion, right?
Too much advancement and this problem won't be addressed.

don’t go too harsh with ur chin, you don’t want to risk looking like a class 3

You think it's too much as it is in the simulation? I thought it wasn't so much since it barely reaches the middle of the nose.

I thought about asking for more bimaxilar advancement rather than genio. But to me, it feels like it would make my mouth even more protruded

You’ll need inframalar and paranasal implants tho either way. Don’t think the protrusion will be that bad with the paired implants

Depends on how his paranasal sits in the after, since you need somewhat a projected paranasal to pull the inframalar implants, you don't want the infraorbitals to stick further or at the same distance as the paranasal.

I think with the bimax the paranasal can be addressed and no need for further implants.

The simulation looks like a very natural result to me.
It really comes down to personal preference whether you want more advancement imo.
