It has been proven by MRI scans of some patients that fillers don't really naturally dissolve, at least for some people , and they can potentially stay in your tissue even for decades, while it's possible to dissolve the most of it with hyaluronidase there's no way to be sure it is completely gone from your body, even micro parts left can attract significant amount of water to that area with time, looksminning you significantly with an heavier and softer look.

They can still look good short term (admitting you go to a skilled injector with an eye for male aesthethics that doesn't make you look like a bogdanoff) but you have to keep in consideration that the remaining hyaluronic acid will ALWAYS attract some amount of water no matter the quantity or type and it will last ages.
They will also migrate given enough time and they will make your skin loose due to the repeated unnatural stretching.
My take is to avoid them like the plague, they are only good for females/grandmas that want their face to look like chipmunks with 0 visible bones, for males always stick to implants/fat graft in the eye area and implants/osteotomies everywhere else.

I guess the real question is what is the quantifiable risk of fillers staying in the body and are particular areas more risk prone than others. Is the risk greater beyond a certain volume?

Almost the entire the sample size were 50+ females who probably irresponsibly used fillers to augment the entire midface.

The HA just lasts much longer than 1-2 yrs imo and we don't know exactly how much even tho we know it dissolves faster if the area is constantly stimulated (lips filler for example are the fastest to dissolve) , people refill the area with more filler that lasts x>2yrs after 1-2 years from last injection and then they are left with much more filler than they think, that now will take more time to dissolve.

There's also a theory that filler never dissolve but visually they look like they dissolved because in reality they have just shredded in smaller parts and migrated somewhere else, however i don't know what to say on this one.

Good points.

I’m just saying that maybe there’s some more nuance to the exact risk profile of fillers and their application.

They are obviously horribly abused to create facial volume.

Fat grafts + implants would likely create a more natural long lasting result. Fillers could supplement them instead of being the workhorse.
