i'm tired to give advice : DJS + Sliding genio. and sharpening your zygos with the right fillers and cog threads and the right surgeon who knows how to do that, same for your lower third definition. If you don't find such a surgeon in your country then go shave some of your zygos, and go get side wing osteomy on top of all that.
