Ortho said remove 4 premolars before braces

After I already had 4 wisdom teeth removed. I'm black with bimaxillary protrusion and very crowded teeth

I went to a female orthodontist and she and her male orthodontist coworker agreed and said that is my only option

I went to seek a male orthodontist in another city for another opinion because I don't trust women and he said, after looking at my X-ray, extractions are my only option as I am too old for expansion. He said expansion was supposed to be done before puberty. I'm 19. Both orthodontic places said if I continue with braces without extractions, my teeth will protrude even more and out the bone

They said I could do any surgery I want even split my face in half and premolar extraction would be the best and superior option

They said my jaw is small and my teeth are big

I told them I'm afraid of bone less, narrow airway, etc and they said it won't happen

The male orthodontist I went to hesitated after I said bone less though. I think I'm gonna have fucking bone loss. What should I do after premolar extraction and braces treatment to avoid and reverse bone loss?

and your regular ortho can't design and put in MARPE, can't perform SARPE since not a surgeon, obviously can't do LF1 Segmentation

and random tooth borne expanders are ASS

Teeth-based expansion (non-skeletal) is very possible in adult age. Skeletal is possible too, although, the results of classic MSE are bit dissapointing, new techniques and tools provide better. Search for "rapid/accelerated orthodontics", with corticotomy, it's when they make a lot of cuts near teeth roots area using piezoelectric (it targets hard tissue while leaving soft tissue mostly intact) and it allows for bigger and way faster movements and better outcomes in general. It would be surprising if it's too rare option in USA.
