Had a talk with my surgeon in regards to custom paranasal/infraorbital implants. He said we can use PEEK/Titanium for infraorbital but recommended silicon/medpor for paranasal, as he said that region naturally is very soft and would be better suited for silicon. Anyone knowledgeable have any opinions?

In terms of appearance, it doesn't matter at all. The only issues you should consider are possible long-term issues (injuries and bone resorption). From my understanding, if it is a properly designed implant that is fully custom, silicone is probably the best option.

What advantages does silicone have over the others? I was under the impression PEEK/Titanium is more stable and have lower infection rates.

What do you mean "more stable"? If the implant is custom fitted and fixed with a screw, it should be very stable, regardless of material. I think infection rates have more to do with the skill of the surgeon during the procedure and aftercare (antibiotics), not the material type.

The advantage of a material like silicone is that it is less likely to break your bones. It could act as a "bumper" rather than a piece that comes loose if you do have an injury severe enough to potentially break a bone.

What surgeon? I am having trouble researching surgeons who offer custom implants.

There hasnt been a paranasal implant ascension posted here once
There have been results but they all end up looking worse and are regretful of it

They’re mostly silicone off the shelf, and don’t blend into the skull, like a custom peek implant would.

Your surgeon is kinda right, because silicone is a good choice but Medpor ain’t.
