I am currently working on a surgical case study.

The patient has a recessed chin but entire lower third is recessed

Is this a bimax or trimax candidate?

I would say bimax, especially for his mandible, but I don't think I can say anything definitely without X-ray scans.

My patient is an interesting case

It is a developmental skeletal mandibular deficiency

The mandible simply did not grow forward enough

Its often inherited (polygenic)

this isn't even only a chin problem he has skeletal mandibular deficiency

so genio alone would be insufficient

i think he needs a maxillary osteotomy first and then genio

it also depends on what bite he has cuz jaw surgery decisions arent based on appearance alone he needs to get in contact with whatever surgeon he needs ASAP

if you know this guy tell him to get an lateral cephalometric x ray first before any procedures happen

You do not treat mandibular deficiency by doing maxilla + genio while skipping the mandible
That would

Leave the mandible retruded, create occlusal and aesthetic imbalance, and be an incorrect skeletal hierarchy

Bimax is necessary, genio is an extra if he wants

Are you being serious?? There's no way he can get away with that chin no amount of advancement will make his side aesthetic if he doesn't fix his chin

Just to clarify I'm not saying he doesn't need bimax, he needs trimax

So trimax + genio

I am a advocate of implants over genio for the chin

Most genio cases dont produce significant MM advancement they usually underperform

He also needs jaw angle implants and under eye fat grafting but that'll cost a lot
I'm not so sure about ccw bimax
