Top has mostly a neurtral canthal tilt. The whole orbit complex needs a couple of degrees of upward rotation, very subtle but would do the trick.

So it cant be skeletal level or hard tissue. And it also doesnt matter for aestethics, they dont have like some kind of xray vision.
