Posterior zygomatic osteotomy

I have been recently looking at all the methods possible to widen the face. As a starter, it would require widening the most posterior zygomatic arch region, as highlighted in the picture:

In theory, you would imagine a ZSO to be able to do this, but in practice, they just target the malar eminence. Here is an example, notice how by widening that region alone you not leave the bizygomatic width untouched, but also manage to get pseudo-asian cheekbones that look completely rounded:

(Keep in mind he had chin wing + genio + orbital floor implants too)

You can see on his scan how the posterior zygomatic arch remains untouched:

I wasn't able to find a SINGLE result where that region is targeted i.e. widens the face

So basically, on a ZSO, the osteotomy site progressively kills the caucasian zygomatic contour and (I assume) makes it closer to what an asian has, and by no one's surprise, they despise and do the reverse surgery

Anyway, in practical terms, I assume the only person who could be able to do such osteotomy is Dr. E, right? Or maybe that is not the case, and there are more people doing and getting it? I'm myself looking for a moderate increase, something like 2-3mm by side so my bizygomatic width achieves human tier.

Inb4 someone says "just get implants" - barely any surgeons use implants that extend far back into the zygomatic arch AND widen it, so good luck with that

They don't because it can't be placed intraorally as normal infraorbital-malar implants are.

I used to question myself: why do literally no Pagnoni/Ramieri patients get wider faces after implants, when half of PSL are narrow cucks?

I think this is the reason why. Same reason why they don't do skull implants i.e. if you don't augment the temples it leaves a weird transition between the zygo arch and the temples.

There has to be a solution

I posted about this once but with intention of narrowing rather than widening.

Chad faces seem to have the malar prominence as the widest point whilst brachy incel faces continue to expand to the posterior zygomatic arch.

You can see this phenomenon in my own incel brachy skull slice below. I think I’m in severe danger of looking Asian if I get infras but in real life the area is obviously very deficient.

The reverse osteotomy is easy, widely done. But it’s really just bone shaving.

Things are usually the way they are for a reason, so they probably dont do it due to some risk with stability ig.

I don’t think that is true. In fact, it’s the opposite: the reason why ZSO for example usually looks like shit is due to the malar eminence being wider in relation to the zygo arch than it was before.

I’m in class rn so I’m gonna tell you to look it up yourself, but look at asian vs caucasian’s zygoma contour. The distinctive caucasian trait is a malar eminence that tapers out to be wider than the zygomatic arch. In a way, this is what most asian cheekbone reduction do - make it more caucasian leaning

In your case, you probably need both the zygo arch AND malar eminence shaved, at least on a first glance, unless the most posterior region is abnormally wide.

The issue is not to be able to do ZSO, is being able to do a cut that is able to widen all the way back. For practical reasons, I imagine you wouldn’t actually do a ZSO as we think technique wise in this case, but idk much here.

I agree, the 16th cranial nerve stimulates the malial emergence to prevent the body from maintaining the supine position look and emulate the effective genital orbitals.

It seems to be even rarer and more complex than getting proper implants. BTW have you seen some good check fillers results? I think it's possible to replicate them with implants, but ofc it shouldn't be off-the-self "apple cheekbones" stuff.

But the result looks kinda like a normal ZSO ngl. Maybe it's the angle and immediate post-OP desk that gives it this effect, but I can't really see a laterally prominent zygo arch on the after, it just looks like a new and more bloated malar eminence jfl.
