Radix augmentation has been practiced since the 1930s using various materials such as excised dorsum, septum, concha, dermis, and fascia. Recently, Sheen has popularized radix augmentation as a "balancing procedure" to minimize hump reduction while preserving dorsal height and achieving a more natural nose appearance. His effective results support this approach by addressing visibility and excessive height issues. Initially, I adopted Sheen’s methods using septal cartilage, but encountered a visibility rate of 30%. I then transitioned to using excised alar cartilage, which reduced visibility to 10%. More recently, I have aligned with Guerrero and Miller in using fascia exclusively for radix augmentation, leading to visibility issues of less than 1%. The following technique outlines grafting methods for either the entire radix area or just the portion between the nasion and the dorsum (the inferior half of the radix area).
