You sure about it?
There are many things you could potentially do ranging from minor surgery to bigger riskier ones but for you it's not that easy because you not have one big failo like a huge nose which a rhino would fix. There is a thread about surgery in botb and it's really good. You should look out for things to improve your lip width mandible and eye area but surgerys in the eye area have high risk so be careful

No, he can't be sure about it because the pictures you have provided don't allow any reliable assessment.
Take a proper front profile and side profile (look up "Frankfort horizontal plane") and post them here. Also, don't fraud or those pics will be useless : don't bite your cheeks in, don't jut, don't tilt your head, don't tense your face/jaw/neck muscles, don't squint, don't make retarded faces ...

Anyway, if I had to take a guess based on the pictures in the OP, I'd say :
a rhinoplasty to rotate the tip of your nose ;
a genioplasty to add lateral & vertical projection, and perhaps width too ;
some upper eyelid work ;
and your temporal peaks seem receded, but that might just be the low res & angle (other pictures needed for proper assessment).

Forget about these morphs. I looked for better pictures in your previous posts, and those 2 will do the trick :

It doesn't matter if these were taken 6 months ago and if you're now sporting a different hair cut. The structural problems remain unchanged.
Namely :
- a very low mouth to nose width ratio, mostly due to an extremely narrow mouth ( @tweaqo @lurking truecel ) ; this is close to impossible to fix, unfortunately. A rhinoplasty (alar plasty) will improve harmony with the mouth but won't fix the whole problem.
- a downswung jaw ; fixable with a bimax. Good call by @Aether_
- a recessed chin ; fixable with a genioplasty (+ bimax).
- an excessively long philtrum ; fixable with a lip lift.
- an excessively upturned nose ; fixable with a bimax and a rhinoplasty.

While you could vastly improve your side profile with the aforementioned surgeries, the narrow mouth is going to hardcap your front profile. If you are really ready to risk it all, do your own research on lateral commissuroplasty ( = lateral expansion of the mouth).

A side profile morph to give you a rough idea of what would be achievable with surgeries :

So genuelly its not a good idea to do genioplasty and rhino because its to risky
